Rheumatic constrictive pericarditis

چکیده
A Case of calcific constrictive pericarditis in ayoung White woman with a convincing history of previous aCute rheumatic fever complicated by a possible Valvular lesion is presented. Cardiac catheterization confirmed the suspicion of significant cardiac compression. Successful pericardiectomy was carried out, but microscopical examination of the excised pericardium failed to demonstrate a cause. In view of the strong past history of acute rheumatic fever and mild mitral insufficiency demonstrated at Cardiac catheterization, the author proposes that the calcific constrictive pericarditis .was of rheumatic origin. A review of the literature on the association between rheumatic infection and Constrictive pericarditis follows.

Constrictive pericarditis in rheumatoid arthritis.
Constrictive pericarditis is an unusual complication of rheumatoid arthritis, and its development appears to be uninfluenced by second-line therapy. Five patients are described who developed the disease, with possibly two more. Medical treatment is ineffective: the treatment of choice is surgical, and the postoperative prognosis is good. The crude prevalence rate for the association is 0.64% fo...

Constrictive pericarditis and pyopericardium with tamponade in a patient and rheumatoid arthritis is described. No fever was recorded despite a litre of pus in the pericardial sac, perhaps because of previous treatment with hydrocortisone. Echocardiography is a diagnostic advance.

Because of uncertainty existing in respect to the etiologic factors in certain cases of constrictive pericarditis, an analysis of 25 proven cases and of additional series from the literature was conducted. It was evident that a fairly consistent clinical syndrome developed, but the factor of myocardial insufficiency deserves as much consideration as actual constriction by the pericardium. The s...

As its name Implies, this disease is distinguished by an inflammatory and cicatricial transformation of the pericardium, trapping the heart in a compressive, inelastic envelope. The deposition of scar tissues may incarcerate the heart, fix the heart and pencardium to unyielding surrounding structures and constrict the great veins and aorta. Clinically, the predominance of any or all of these fe...
